We try to see all patients on an appointment basis and ask that you call in advance to schedule appointments. Please call our office at (805) 688-2600 to schedule an appointment. Examinations, follow-up visits, and routine appointments are available Monday through Friday, and What to Bring to Your Appointment - Your insurance card
- List of prescriptions and/or over-the-counter medication, including dose and frequency
- Information about your medical history
We make every effort to honor our time commitments and ask that you do the same for us. If you cannot keep a scheduled appointment, please call us 24 hours in advance to cancel or reschedule. We reserve the right to bill patients who repeatedly fail to show for appointments or do not provide 24 hours notice of cancellations.
