Billing
You will be billed by statement once each month in the cycle immediately following your last date of service. Charges shown on the statement are due and payable when received. If other financial arrangements are necessary, please discuss these with the office personnel prior to receiving the service.
Insurance and HMO Plans
We accept patients whose care is covered by insurance or HMO plans, Medicare and other insurers. Please remember the patient is responsible for payment of co-pays and deductibles specified by their plan coverage. When recommended care is not covered by the patient's plan, the patient is responsible for payment of the non-covered charges.
Insurance and Claim Forms
We are pleased to assist in filing claim forms for your insurance reimbursement or direct payment when it is required. We will file Medicare, Medicare supplement plans, and other insurance claims automatically and electronically whenever possible. Please remember that all charges, including deductibles and co-pays for covered services not paid by the insurance company, are the patient’s responsibility.
Fees and Payments
Every effort is made to limit the costs of your medical care, without compromising your service. We ask that payment be made at the time of service. All co-pays and deductibles must be made at the time of the service. We are pleased to accept cash, local checks, Visa, MasterCard, or American Express.
